La infusión a través del hueso comienza a utilizarse en 1922. En los menores de 6 años los sitios ideales son la tibia proximal, distal y fémur distal. Es una técnica extremadamente fácil que puede ser practicada de forma rápida, por donde se pueden administrar todos los medicamentos necesarios para la reanimación. Se indica en situaciones donde no se logra un acceso venoso después de 3 intentos o pasados 90 min. La técnica se contraindica en la celulitis u osteomielitis en el sitio de aplicación, la osteopetrosis y en la osteogénesis imperfecta. A través de esta vía pueden administrarse múltiples sustancias. El índice de complicaciones de esta técnica es muy bajo. Los accesos vasculares en los niños deben establecerse de forma escalonada e iniciarse con las técnicas menos invasivas]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The vascular access together with the permeabilization of the airway and the adequate ventilation are the most important procedures in the emergency service. Intraosseous infusion began to be used in 1922. In under 6 years-old children, the ideal sites are proximal and distal tibia and distal femur. It is an extremely easy technique that may be quickly applied, through which all the necessary drugs for resuscitation can be administered. It is indicated when there is no vein access after three attempts or 90 minutes elapsed. However, it is contraindicated if cellulitis or osteomyelitis is present in the site of application, or in the case of osteopetrosis and imperfect osteogenesis. Multiple substances may be administered through this route. The rate of complications is very low. The vascular access sites should be progressively set in children and applied with the less invasive techniques.]]></p></abstract>

La infusi&oacute;n a trav&eacute;s del  hueso comienza a utilizarse en 1922. En los menores de 6 a&ntilde;os los sitios  ideales son la tibia proximal, distal y f&eacute;mur distal. Es una t&eacute;cnica  extremadamente f&aacute;cil que puede ser practicada de forma r&aacute;pida, por  donde se pueden administrar todos los medicamentos necesarios para la reanimaci&oacute;n.  Se indica en situaciones donde no se logra un acceso venoso despu&eacute;s de  3 intentos o pasados 90 min. Los accesos vasculares en los ni&ntilde;os deben establecerse de forma escalonada  e iniciarse con las t&eacute;cnicas menos invasivas    <br>     <br> <i>DeCS:</i> INFUSIONES  INTRAOSEAS/m&eacute;todos; URGENCIAS MEDICAS; PREESCOLAR</p>    <p>El acceso vascular  conjuntamente con la permeabilizaci&oacute;n de la v&iacute;a a&eacute;rea y la  ventilaci&oacute;n adecuada son los procedimientos m&aacute;s importantes en el  servicio de urgencias. Por tal motivo los servicios de urgencias deben contar  con personal adiestrado en las t&eacute;cnicas de acceso vascular. En los ni&ntilde;os  las caracter&iacute;sticas anat&oacute;micas y fisiol&oacute;gicas tambi&eacute;n  influyen en la permeabilizaci&oacute;n vascular, como es la presencia de un cuello  corto, que dificulta el acceso a venas centrales, yugulares y subclavia; en el  reci&eacute;n nacido, la v&iacute;a umbilical es un acceso r&aacute;pido y f&aacute;cil,  pero s&oacute;lo en las primeras 2 semanas de vida, y aun as&iacute; actualmente  se describen secuelas debido a la utilizaci&oacute;n de esta v&iacute;a venosa.<span class="superscript">1</span>    <br>      <br> Los ni&ntilde;os peque&ntilde;os tienen m&aacute;s grasa subcut&aacute;nea  y se dificulta la localizaci&oacute;n de las venas perif&eacute;ricas en las extremidades.  Desde el punto de vista fisiol&oacute;gico, en los ni&ntilde;os los cambios hemodin&aacute;micos  son m&aacute;s dram&aacute;ticos e influyen en el acceso a una v&iacute;a vascular,  cuando disminuye el volumen circulante, se produce constricci&oacute;n de los  vasos perif&eacute;ricos para mantener un gasto card&iacute;aco adecuado, lo que  hace imposible la canalizaci&oacute;n de una v&iacute;a.<span class="superscript">1-3</span>    <br>      ]]></body>
<body><![CDATA[<br> Todo lo antes expuesto resalta la importancia del acceso a una v&iacute;a  alternativa que permita la administraci&oacute;n de fluidos y f&aacute;rmacos  ante una situaci&oacute;n emergente, como la intra&oacute;sea. El objetivo de  esta revisi&oacute;n es resaltar la importancia de esta v&iacute;a en pediatr&iacute;a.</p><h4>Aspectos  hist&oacute;ricos<span class="superscript">1,2</span></h4>    <p>La infusi&oacute;n  a trav&eacute;s de hueso comienza a utilizarse en 1922 en animales de experimentaci&oacute;n,  y en 1934 en humanos para tratar la anemia perniciosa, aunque a partir de 1940  su uso se difundi&oacute; ampliamente para transfundir pacientes con granulocitopenia.      <br>     <br> En los a&ntilde;os 1942 y 1943 se demostr&oacute; la absorci&oacute;n  instant&aacute;nea de cada v&iacute;a y se concluy&oacute; que era equivalente  a la intravenosa.    <br>     <br> En 1917 <i>Hernald</i> analiz&oacute; datos de 982  infusiones con &eacute;xito en 964 casos y en 1954 se utiliz&oacute; la cresta  il&iacute;aca para la infusi&oacute;n de f&aacute;rmacos con buenos resultados.  A partir de la d&eacute;cada de 1950 se comienza a utilizar otras v&iacute;as  de infusi&oacute;n y disminuye el uso de la v&iacute;a intra&oacute;sea hasta  casi desaparecer. Revive nuevamente el inter&eacute;s por esta v&iacute;a en 1977  a causa de trabajos con venograf&iacute;a intra&oacute;sea. En 1984 <i>Rosetti</i>  y otros publicaron un estudio de 3 a&ntilde;os en ni&ntilde;os bajo reanimaci&oacute;n  cardiopulmonar, donde observaron que en el 6 % de los casos reanimadores experimentados  no lograron el acceso vascular, y en el resto de los casos el tiempo promedio  para lograrlo fue de 7,8 min y en el 25 % de los pacientes se necesitaron 10 min  o m&aacute;s para lograrlo. Estos mismos autores en 1985 revisaron 4 359 intentos  con solo el 2,9 % de fallos. Actualmente muchos estudios han confirmado la eficacia  y seguridad de esta v&iacute;a. La m&eacute;dula permite una entrega r&aacute;pida  y completa de l&iacute;quidos y medicamentos.</p><h4>Sitios anat&oacute;micos<span class="superscript">3-5</span></h4>    <p>En  los menores de 6 a&ntilde;os los sitios ideales son la tibia proximal, distal  y f&eacute;mur distal, los lugares alternos son cresta il&iacute;aca y troc&aacute;nter  mayor. En los mayores de 6 a&ntilde;os la cresta il&iacute;aca y el estern&oacute;n  constituyen los sitios ideales, y la tibia distal y el troc&aacute;nter mayor  los sitios alternos.</p><h4>T&eacute;cnica para la infusi&oacute;n intra&oacute;sea<span class="superscript">1-3,6</span></h4>    <p>Previa  asepsia y antisepsia y colocaci&oacute;n de campos est&eacute;riles en el sitio  de inserci&oacute;n, se localiza el lugar. En &eacute;l, casi de elegirse el punto  tibial proximal, 2 cm por debajo de la tuberosidad anterior y 1 cm hacia la cara  medial, se introduce el tr&oacute;car, perpendicularmente al platillo tibial,  con cierta orientaci&oacute;n distal, para no afectar el n&uacute;cleo de osificaci&oacute;n.  Cuando se alcanza el periostio, es necesario imprimirle a la aguja un movimiento  rotatorio, hasta sentir una disminuci&oacute;n de la resistencia, lo que indica  que se ha llegado al canal medular que se confirma con: a) aspiraci&oacute;n de  material medular, b) ausencia de movilidad de la aguja, c) flujo libre de l&iacute;quido  y d) ausencia de aumento de partes blandas.    <br>     <br> Para el f&eacute;mur distal  de inserci&oacute;n la t&eacute;cnica se efect&uacute;a en la l&iacute;nea media,  2 &oacute; 3 cm por encima de los c&oacute;ndilos femorales, con sentido cef&aacute;lico  para evitar la lesi&oacute;n del platillo de crecimiento. Las agujas para el acceso  intra&oacute;seo son: aguja para punci&oacute;n lumbar, aguja para aspirado esternal,  y el dispositivo SAVE (Esternal Access Venous Entry).</p><h4>Ventajas<span class="superscript">2</span></h4><ul>      ]]></body>
<body><![CDATA[<li>Es una t&eacute;cnica extremadamente f&aacute;cil.    <br> </li>    <li>Puede ser  practicada de forma r&aacute;pida.    <br> </li>    <li>Se pueden administrar todos los  medicamentos necesarios para la reanimaci&oacute;n.    <br> </li>    <li>Permite la infusi&oacute;n  de grandes cantidades de l&iacute;quidos.    <br> </li>    <li>Las sustancias administradas  alcanzan la circulaci&oacute;n central r&aacute;pidamente.    <br> </li>    ]]></body>
<body><![CDATA[<li>Los sitios  de accesos son distantes a las &aacute;reas donde se practican las maniobras de  reanimaci&oacute;n.    <br> </li>    <li>Se asocia a pocas complicaciones.</li>    </ul><h4>Indicaciones<span class="superscript">6-10</span></h4>    <p>Est&aacute;  indicada en situaciones donde no se logra un acceso venoso despu&eacute;s de 3  intentos o pasados 90 min.</p><h4>Contraindicaciones<span class="superscript">1,2</span></h4>    <p>La  t&eacute;cnica se contraindica en la celulitis u osteomielitis en el sitio de  aplicaci&oacute;n, la osteopetrosis y en la osteogenesis imperfecta.    <br> Tampoco  se recomiendan intentos repetidos en el mismo sitio, pues las punciones m&uacute;ltiples  reducen la eficacia.</p><h4>Sustancias que pueden administrarse por v&iacute;a  intra&oacute;sea.<span class="superscript">1,2,5,9,11</span></h4>    <p>L&iacute;quidos:  Soluci&oacute;n salina hipert&oacute;nica, ringer lactado, dextr&aacute;n 40,  sangre total.    <br>     <br> F&aacute;rmacos: Antibi&oacute;ticos, antitoxinas, atracuronio,  atropina, calcio, dexametasona, diazep&aacute;n, diazoxido, digoxina, dobutamina,  dopamina, adrenalina, heparina, isuprel, noradrenalina, lidoca&iacute;na, lorazepan,  fenito&iacute;na, propanolol, bicarbonato de sodio, tiopental s&oacute;dico, pentotal,  succinilcolina, insulina.</p><h4>Complicaciones<span class="superscript">1,3,7,10</span></h4>    ]]></body>
<body><![CDATA[<p>El  &iacute;ndice de complicaciones de esta t&eacute;cnica es bajo, pero las halladas  son:    <br>     <br> &nbsp;&nbsp;I. Locales: Extravasaci&oacute;n de l&iacute;quidos  y f&aacute;rmacos por una mala colocaci&oacute;n de la aguja, celulitis local,  osteomielitis, fracturas, &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;lesi&oacute;n del platillo  de crecimiento.    <br> II. Generales: Embolismo graso y &oacute;seo, depresi&oacute;n  medular, sepsis, perforaci&oacute;n de estern&oacute;n seguida de mediastinitis,  &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;hidrot&oacute;rax, perforaci&oacute;n de grandes  vasos, muerte.</p><h4>Prioridades en los accesos vasculares<span class="superscript">1,11</span></h4>    <p align="left">Los  accesos vasculares en los ni&ntilde;os deben establecerse de forma escalonada  e iniciarlos con las t&eacute;cnicas menos invasivas. Las punciones venosas percut&aacute;neas  se deben intentar antes que las disecciones venosas, de igual forma las venas  distales perif&eacute;ricas de las extremidades antes que las proximales. En primer  t&eacute;rmino la venopunci&oacute;n percut&aacute;nea de las venas perif&eacute;ricas  distales de las extremidades, o en el cuero cabelludo en lactantes; de no ser  posibles se intenta en venas m&aacute;s proximales de las extremidades y por &uacute;ltimo  la canalizaci&oacute;n de venas centrales.    <br>     <br> Ante situaciones emergentes  la cateterizaci&oacute;n venosa percut&aacute;nea de vasos perif&eacute;ricos  es imposible en 90 min o despu&eacute;s de 3 intentos, y entonces es necesario  recurrir a otra v&iacute;a, que en los menores de 6 a&ntilde;os es la intra&oacute;sea  la de elecci&oacute;n y en el mayor la vena femoral. De no lograrse la canalizaci&oacute;n,  las venas femoral y safena son las recomendadas en caso de PCR, ya que no interfieren  con las maniobras de reanimaci&oacute;n. Si no hay arresto cardiorrespiratorio  se recomienda la yugular externa seguida por la interna y posteriormente la subclavia  (fig).</p>    <p align="center">    <br> <a href="/img/revistas/ped/v75n3/f0111303.jpg"><img src="/img/revistas/ped/v75n3/f0111303.jpg" width="377" height="324" border="0"></a></p>
