Fueron aplicadas las siguientes pruebas estadísticas: medias, desviación estándar, mínimo, máximo y prueba de correlación-regresión e hipótesis con una confiabilidad de 95 % (alfa = 0,05). El 38,5 % de los lesionados correspondieron al rango entre 21 y 30 años de edad y predominó el sexo masculino (88,5 %). Los accidentes del tránsito constituyeron la causa más frecuente de lesiones (38,5 %); las lesiones ortopédicas fueron las de mayor incidencia (26,4 %) y se encontraron complicaciones en el 38,4 % de la muestra. Las proteínas totales, las albúminas y las globulinas gamma sufrieron disminución significativa a las 72 horas de ocurridas las lesiones, esta depresión se hizo más intensa en los fallecidos y se constató que existe correlación entre las variables estudiadas y los índices predictivos.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[A prospective study was perfomed on 52 traumatized patients, and the correlation between tha values of the predictive index used and the results obtained with the protein fractions studied at admission and on the 3rd and 7th days was established. The following statistical tests were applied: median, standard deviation, minimum, maximum, and the correlation-regresion test, and hypothesis with a confidence interval of 95 % (alpha = 0.05). 38.5 % of the injured patients were within the rank of 21 to 30 years of age and males were found to be predominant (88.5 %). Traffic accidents accounted for the most frequent cause of lesions (38.5 %); orthopaedic lesions were found to have the greatest incidence (26.4 %), and complications were found in 38.4 % of the sample. Total proteins, albumins, and gamma globulins were found to be significantly reduced 72 hours after <a name=QuickMark></a>the lesions; this reduction was more intense in subjects who died. A correlation between the variables studied and the predictive indices was evidenced.]]></p></abstract>

Las prote&iacute;nas totales, las alb&uacute;minas y las  globulinas gamma sufrieron disminuci&oacute;n significativa a las 72 horas de  ocurridas las lesiones, esta depresi&oacute;n se hizo m&aacute;s intensa en los  fallecidos y se constat&oacute; que existe correlaci&oacute;n entre las variables  estudiadas y los &iacute;ndices predictivos.     <P><I>Descriptores DeCS:</I> HERIDAS  Y LESIONES/metabolismo; PROTEINAS/metabolismo; ALBUMINAS/metabolismo; GLOBULINAS/metabolismo;  PUNTAJE DE GRAVEDAD DE LA LESION; ESTUDIOS PROSPECTIVOS.</DIR></DIR>Los traumatismos  son la principal causa de muerte durante las 4 primeras d&eacute;cadas de la vida  y como causa global de fallecimientos s&oacute;lo son superados por el c&aacute;ncer  y la ateroesclerosis.<SUP>1</SUP>     <P>En la b&uacute;squeda de par&aacute;metros  objetivos destinados a establecer pron&oacute;sticos, han sido creados y perfeccionados  m&uacute;ltiples &iacute;ndices predictivos, desde los primeros que intentaron  estudiar el pro-n&oacute;stico de los pacientes quemados, hasta los m&aacute;s  utilizados en la actualidad.<SUP>2-6</SUP>     <P>Los traumatismos producen en el  paciente m&aacute;s de 200 mediadores que se pueden clasificar en hormonas, citocinas  y mediadores lip&iacute;dicos. Este proceso forma parte de la reacci&oacute;n  inflamatoria normal; pero cuando se hace generalizado y autodestructivo se denomina:  respuesta inmuno-humoral, hiperintensa y no controlada o respuesta inflamatoria  maligna, lo que provoca da&ntilde;o org&aacute;nico progresivo. En estos casos  los mediadores se producen de forma an&aacute;rquica, cantidad excesiva y por  un tiempo demasidado prolongado, y se traduce el proceso como inmunodepresi&oacute;n,  con la consecuente disminuci&oacute;n de los componentes del sistema inmunol&oacute;gico.      <P>Al considerar que tanto las citocinas como una parte fundamental de los componentes  de la respuesta inmune, estaban formados por p&eacute;ptidos, se traz&oacute;  un proyecto de estudio metab&oacute;lico e inmunol&oacute;gico del paciente traumatizado,  cuyo primer producto lo constituy&oacute; este trabajo. <H4> M&Eacute;TODOS</H4>Se  realiz&oacute; un estudio prospectivo en 52 traumatizados admitidos en el hospital  de traumatolog&iacute;a "Magdalena de las Salinas" (HTMS), Distrito Federal, M&eacute;xico;  desde julio de 1993 a enero de 1994.     <P>Para seleccionar la muestra se incluyeron  los lesionados con valores de Injury Severity Score (ISS) mayor de 8, seg&uacute;n  &iacute;ndice predictivo de Susan Baker.     ]]></body>
<body><![CDATA[<P>Se les hallaron los valores de trauma  score (TS) y la probabilidad de sobrevida (PS) seg&uacute;n &iacute;ndice predictivo  (TRISS). <body><![CDATA[<CENTER>0,3</CENTER></TD></TR>  <TR> <TD VALIGN=TOP WIDTH="33%">- Globulinas alfa 2</TD><TD VALIGN=TOP WIDTH="33%">      <CENTER>0,4</CENTER></TD><TD VALIGN=TOP WIDTH="33%">     <CENTER>0,7</CENTER></TD></TR>  <TR> <TD VALIGN=TOP WIDTH="33%">- Globulinas beta</TD><TD VALIGN=TOP WIDTH="33%">      <CENTER>0,5</CENTER></TD><TD VALIGN=TOP WIDTH="33%">     <CENTER>0,8</CENTER></TD></TR>  <TR> <TD VALIGN=TOP WIDTH="33%">- Globulinas gamma</TD><TD VALIGN=TOP WIDTH="33%">      <CENTER>0,8</CENTER></TD><TD VALIGN=TOP WIDTH="33%">     <CENTER>1,2</CENTER></TD></TR>  </TABLE></CENTER>Fueron realizadas pruebas de significaci&oacute;n estad&iacute;stica  con ayuda del procesador estad&iacute;stico MICROSTAT y se construyeron tablas  con los resultados obtenidos (medias, desviaci&oacute;n est&aacute;ndar, m&iacute;nimo,  m&aacute;ximo).     <P>Mediante el estudio de la prueba de varianza, se pudo despu&eacute;s  ordenar las muestras por cada 1 de los &iacute;ndices predictivos y los valores  medios de las variables de las fracciones prot&eacute;icas estudiadas.     <P>Fueron  aplicadas pruebas de correlaci&oacute;n-regresi&oacute;n e hip&oacute;tesis con  una confiabilidad de 95 % (alfa = 0,05) para comprobar si existi&oacute; relaci&oacute;n  entre cada uno de los &iacute;ndices predictivos (ISS, TS y PS) y los resultados  obtenidos en el estudio cin&eacute;tico de las prote&iacute;nas. <H4> RESULTADOS</H4>La  media de edad de los lesionados fue de 39,9 a&ntilde;os, que oscilaron en el rango  de 15 a 81. El sexo predominante fue el masculino (88,5 %). Los accidentes de  tr&aacute;nsito constituyeron el mecanismo de lesi&oacute;n m&aacute;s frecuente  (38,5 %) y si se agrupan las extremidades y los cinturones escapular y pelviano  se observ&oacute; que &eacute;sta constituy&oacute; la regi&oacute;n m&aacute;s  vulnerable a las lesiones (26,4 %) seguida del t&oacute;rax (25,6 %).     <P>Se complicaron  en esta serie 30 pacientes (38,4 %) y la complicaci&oacute;n m&aacute;s frecuente  fue la neumopat&iacute;a inflamatoria (15,6 %).     ]]></body>
<body><![CDATA[<P>Al hacer un an&aacute;lisis  del comportamiento de las fracciones prot&eacute;icas (tabla 1), se constataron  que las prote&iacute;nas totales, las alb&uacute;minas y las globulinas gamma,  partieron de un valor cercano al l&iacute;mite inferior del rango de normalidad,  seg&uacute;n valores de referencia en las primeras 24 horas de ocurrido el accidente,  los que descendieron de forma significativa a valores subnormales a las 72 horas  y tienen una recuperaci&oacute;n a los 7 d&iacute;as al ascender hasta cifras  cercanas a las iniciales, pero sin demostrar a&uacute;n una recuperaci&oacute;n  total, excepto las globulinas gamma.     <P>La correlaci&oacute;n entre el ISS las  fracciones prot&eacute;icas y la morbimortalidad, mostr&oacute; que las prote&iacute;nas  totales y las alb&uacute;minas (tabla 2) sufren depresi&oacute;n estad&iacute;sticamente  significativa a las 72 horas, y se present&oacute; en todos los rangos de ISS,  cifras inferiores a la normalidad, hasta recuperar valores normales, excepto los  que presentaron cifras de ISS superiores a 40, en quienes se mantiene la misma  depresi&oacute;n.     <P>Las globulinas beta y gamma mostraron disminuci&oacute;n  no significativa a las 72 horas en todos los rangos de ISS, las globulinas alfa  1 un discreto aumento y mantuvieron inestabilidad en su comportamiento las globulinas  alfa 2.     <P>A medida que aumentaron los valores de ISS, las globulinas alfa 1 mostraron  cifras mayores, las alfa 2 disminuyeron en las 3 muestras, las beta s&oacute;lo  disminuyeron a las 72 horas y las gamma se comportaron de forma inestable, todas  sin significaci&oacute;n estad&iacute;stica.     <P>Al hacer un an&aacute;lisis similar  con el TS (tabla 3), se observ&oacute; que a medida que disminuyeron los valores  de TS, se reportan cifras menores de prote&iacute;nas totales y alb&uacute;minas  durante las primeras 24 horas y a las 72 horas de producido el traumatismo, hecho  que se acompa&ntilde;a de una mayor incidencia de complicaciones y una mayor mortalidad,  esta dependencia fue estad&iacute;sticamente significativa, lo que demuestra una  mayor envergadura fisiol&oacute;gica del trauma.     <CENTER>TABLA 1. <I>Cin&eacute;tica  de las fracciones pret&eacute;icas despu&eacute;s del trauma (valores promedios)</I></CENTER><TABLE BORDER CELLPADDING=5 >  <TR> <TD VALIGN=TOP WIDTH="10%">Electroforesis de prote&iacute;nas (gr/L)</TD><TD VALIGN=TOP COLSPAN="3" WIDTH="30%">      <CENTER>Vivos</CENTER></TD><TD VALIGN=TOP COLSPAN="3" WIDTH="30%">     <CENTER>Fallecidos</CENTER></TD><TD VALIGN=TOP COLSPAN="3" WIDTH="30%">      <CENTER>Total</CENTER></TD></TR> <TR> <TD VALIGN=TOP WIDTH="10%">&nbsp;</TD><TD VALIGN=TOP WIDTH="10%">      <CENTER>24 h</CENTER></TD><TD VALIGN=TOP WIDTH="10%">     ]]></body>

<body><![CDATA[<CENTER>0,9</CENTER></TD></TR>  </TABLE></CENTER>    <CENTER>p &lt; 0,05</CENTER>    <P>Complicados y fallecidos: Porcentajes  que representan del n&uacute;mero de lesionados por rangos de TS.     <P>Las globulinas  beta y gamma mantuvieron el patr&oacute;n general de disminuci&oacute;n en casi  todos los rangos de TS, las alfa 2 se comportaron de forma inestable y las alfa  1 mostraron un ligero incremento en la segunda muestra.     <P>Al establecer la correlaci&oacute;n  entre la probabilidad de sobrevida (PS - TRISS), las difernetes fracciones prot&eacute;icas  y la morbimortalidad (tabla 4), se observ&oacute; que las prote&iacute;nas totales  y las alb&uacute;minas mantienen el patr&oacute;n de depresi&oacute;n a las 72  horas en casi todos los rangos de PS y que decrecieron sus valores a medida que  disminuy&oacute; la PS, lo que es significativo a las 72 horas. Esta dismunici&oacute;n  se acompa&ntilde;&oacute; de incremento en el porcentaje de complicados y fallecidos  por grupos. <body><![CDATA[<CENTER>0,9</CENTER></TD><TD VALIGN=TOP WIDTH="8%">      <CENTER>1,2</CENTER></TD></TR> </TABLE>    <CENTER>p &lt; 0,05</CENTER>    <P>PS = probabilidad  de supervivencia seg&uacute;n &iacute;ndice predictivo TRISS.     <P>Complicados y  fallecidos: Porcentajes que representan del n&uacute;mero de lesionados por rangos  de PS.     <P>En la distribuci&oacute;n por rangos de PS s&oacute;lo las prote&iacute;nas  totales, las alb&uacute;minas y globulinas beta y gamma mostraron el patr&oacute;n  general de depresi&oacute;n a las 72 ho-ras y excepto las globulinas beta, el  resto sufri&oacute; disminuci&oacute;n a medida que es menor la PS a las 24, 72  horas y 7 d&iacute;as, hecho que a pesar de no ser significativo para las globulinas,  va acompa&ntilde;ado de un evidente incremento de la morbimortalidad que ascendi&oacute;  a 75 % de los complicados y 37,5 % de fallecidos para el grupo de PS menor de  81 %.     <P>Se constat&oacute; que a medida que aumentaron los valores de ISS, disminuyeron  las cifras de TS, y que la PS es menor, lo que demostr&oacute; mayor gravedad  anat&oacute;mico-fisiol&oacute;gica y adem&aacute;s influy&oacute; la edad como  factor de riesgo. Sufrieron disminuci&oacute;n las prote&iacute;nas totales y  las alb&uacute;minas significativamente durante las primeras 24 y 72 horas; se  destacaron en este sentido las globulinas gamma, estrato donde se encuentran las  inmunoglobulinas, que aunque sin significaci&oacute;n estad&iacute;stica, mostraron  evidente disminuci&oacute;n, todo lo cual actu&oacute; sobre el sistema inmunol&oacute;gico  y provoc&oacute; depresi&oacute;n de una parte importante de sus componentes.      <P>En el estudio se hizo evidente que existe dependencia entre las fracciones  prot&eacute;icas y los &iacute;ndices predictivos estudiados, esto se comprob&oacute;  con los valores de los coeficientes de correlaci&oacute;n obtenidos y los diagramas  de dispersi&oacute;n, donde los puntos se acercan lo suficiente a la l&iacute;nea  de regresi&oacute;n. Esta correlaci&oacute;n fue planteada por algunos investigadores<SUP>15-17</SUP>.  En este sentido Kapur<SUP>12</SUP>, como se hizo referencia en este trabajo, demostr&oacute;  que ocurre disminuci&oacute;n del complemento s&eacute;rico C-3 en traumatizados  con valores de ISS mayores o iguales a 12 y Meritchie<SUP>14</SUP> constat&oacute;  disminuci&oacute;n significativa de las inmunoglobulinas G y M en un grupo de  lesionados portadores de traumatismos cerrados con cifras de ISS, mayores o iguales  a 20. Otros investigadores han evidenciado el papel inmunomodulador de ciertos  p&eacute;ptidos y su relaci&oacute;n con los traumatismos.<SUP>15-20</SUP>     <P>Una  vez finalizado este estudio podemos concluir planteando que la disminuci&oacute;n  significativa de prote&iacute;nas totales, alb&uacute;minas y globulinas gamma  a las 72 horas de ocurridas las lesiones y la correlaci&oacute;n significativa  de las 2 primeras con un aumento en los valores de ISS y una disminuci&oacute;n  de los valores de TS y PS produce en el paciente complicaciones y es causa de  fallecimientos. <H4> SUMMARY</H4>A prospective study was perfomed on 52 traumatized  patients, and the correlation between tha values of the predictive index used  and the results obtained with the protein fractions studied at admission and on  the 3<SUP>rd</SUP> and 7<SUP>th</SUP> days was established. The following statistical  tests were applied: median, standard deviation, minimum, maximum, and the correlation-regresion  test, and hypothesis with a confidence interval of 95 % (alpha = 0.05). 38.5 %  of the injured patients were within the rank of 21 to 30 years of age and males  were found to be predominant (88.5 %). Traffic accidents accounted for the most  frequent cause of lesions (38.5 %); orthopaedic lesions were found to have the  greatest incidence (26.4 %), and complications were found in 38.4 % of the sample.  Total proteins, albumins, and gamma globulins were found to be significantly reduced  72 hours after&nbsp;<A NAME="QuickMark"></A>the lesions; this reduction was more  intense in subjects who died.
