<body><![CDATA[<p>&nbsp;</p>     <p>&nbsp;</p> <hr>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>RESUMEN </b>    </font></p>     <p> <font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Introducci&#243;n:</b>    Diversos estudios han demostrado la presencia de anticuerpos anti C1q en enfermos    con lupus eritematoso sist&#233;mico, aunque su participaci&#243;n en la fisiopatogenia    de la enfermedad no se encuentra totalmente esclarecida. El C1q es fundamental    en la eliminaci&#243;n de los inmunocomplejos y de los debris celulares apopt&#243;ticos,    conocida fuente de auntoant&#237;genos, y en el mantenimiento del endotelio    vascular. Existe una amplia variedad de autoanticuerposcomo los anticuerpos    antinucleares, los anti DNA de doble y simple cadena que junto a los anticuerpos    anti C1q son usados como marcadores de diagn&#243;stico en pacientes con esta    enfermedad. <b>    <br>   Objetivo</b>: Determinar la prevalencia de los anticuerpos anti C1q en la fase    activa de la enfermedad. <br/>   <b>M&#233;todos</b> : Se determinaron los niveles de los anticuerpos anti C1q,    mediante un ensayo inmunoenzim&#225;tico en el suero de 64 enfermos: 20 con    lupus eritematoso sist&#233;mico en fase activa seg&#250;n el &#237;ndice de    actividad del lupus eritematoso sist&#233;mico, 44 con otras enfermedades autoinmunes    reumatol&#243;gicas y 30 donantes sanos. El estudio se realiz&#243; en el per&#237;odo    comprendido entre el 2013 al 2015 y todos los ensayos inmunoenzim&#225;ticos    pertenecen a la firma comercial Orgentec. <br/>   <b>Resultados:</b> Se detectaron anticuerpos anti C1q en el suero de 8 de los    20 enfermos con lupus en fase activa (40 %), se demostr&#243; la presencia de    anticuerpos anti C1q &#160;en 9 de los que ten&#237;an otras enfermedades reumatol&#243;gicas    autoinmunes (20,5 %) y en 4 de los individuos sanos (13,3 %). <br/>   <b>Conclusiones</b> : estos resultados sugieren la posible participaci&#243;n    de los anticuerpos anti C1q en el da&#241;o existente en la fase activa de la    enfermedad. </font></p>     <p> <font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Palabras clave</b>    : anti-C1q, anti-DNA ds, lupus eritematoso sist&#233;mico, autoanticuerpos.    </font></p> <hr>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>ABSTRACT</b>    </font></p>     <p> <font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Introduction</b>    : Several studies have demonstrated the presence of anti-C1q antibodies in patients    with systemic lupus erythematosus, although their participation in the pathogenesis    of the disease is not fully clarified. The C1q plays a role in removing immune    complexes and apoptotic cell debris, known source auntoantigens, and maintenance    of the vascular endothelium. There is a wide variety of autoantibodies as antinuclear    antibodies, anti double and single stranded DNA alongside the anti-C1q antibodies    are used as diagnostic markers in patients with this disease. <br/>   <b>Objective</b> : To determine the prevalence of antibodies to C1q in the active    phase of the disease. <br/>   <b>Methods</b> : The levels of anti-C1q antibodies were determined by an enzyme-linked    immunosorbent assay (Orgentec) in the serum of 64 patients: 20 with systemic    lupus erythematosus in active phase according to the index of activity of systemic    lupus erythematosus, 44 with other rheumatic autoimmune diseases, and 30 healthy    donors. <body><![CDATA[<p> <font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b><font size="3">INTRODUCCI&#211;N</font></b>    </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> El sistema complemento    es muy importante en la respuesta inmune innata. El C1q es el primer componente    involucrado en la cascada de activaci&#243;n por la v&#237;a cl&#225;sica y    participa en la solubilizacion y eliminaci&#243;n de los inmunocomplejos, en    la remoci&#243;n de los debris celulares apoptoticos, conocida fuente de autoant&#237;genos,    y en el mantenimiento de la integridad del endotelio vascular<sup>1-3</sup>.    </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> La deficiencia    hereditaria de la fracci&#243;n C1q del complemento es considerado un fuerte    riesgo gen&#233;tico de desarrollar manifestaciones cut&#225;neas parecidas    al lupus eritematoso sist&#233;mico (LES), as&#237; como una alta predisposici&#243;n    a las infecciones bacterianas <sup>4</sup>. Las deficiencias de los primeros    componentes de la v&#237;a cl&#225;sica del complemento como el C4 y la lectina    de uni&#243;n a&#160; manosa (MBL), reactante de fase aguda y responsable de    la activaci&#243;n del complemento por la v&#237;a de las lectinas, han emergido    como posibles causas de susceptibilidad al LES que es el prototipo de enfermedad    autoinmune caracterizada por una desregulaci&#243;n de c&#233;lulas T y B que    provoca la producci&#243;n de autoanticuerpos, la deposici&#243;n de inmunocomplejos,    la infiltraci&#243;n de c&#233;lulas mononucleares y finalmente el &#160;da&#241;o    tisular<sup>5, 6</sup>. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> En la patog&#233;nesis    de las enfermedades autoinmunes est&#225;n involucrados casi un centenar de    anticuerpos dirigidos contra un gran n&#250;mero de autoant&#237;genos intracelulares,    tanto nucleares como citoplasm&#225;ticos, entre los que se encuentran los anticuerpos    antinucleares&#160; (ANA), anticuerpos anti DNA de &#160;doble y simple cadena,    as&#237; como contra algunas ribonucleoproteinas que tienen la propiedad de    ser extra&#237;bles en soluci&#243;n salina&#160; (ENA)&#160; con diferentes    especificidades,&#160; anti SS-A (SS-52, SS-60), anti SS-B, anti Sm, anti RNP,    anti SCL- 70, antinucleosoma y m&#225;s recientemente los anticuerpos anti C1q<sup>7,    8</sup>. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Ya en la d&#233;cada    de los 80 del siglo pasado, se publicaron reportes de la presencia de anticuerpos    anti C1q en el suero de pacientes con LES <sup>9-11</sup>, pero tambi&#233;n    han sido estudiados en una amplia variedad de condiciones autoinmunes como la    vasculitis urticarial hipocomplement&#233;mica<sup>12</sup>, en el Sj&#246;gren<sup>13</sup>,    en la enfermedad <em>Beh&#231;et</em><sup>14</sup>, glomerulonefritis membrano    proliferativa<sup>15</sup>, as&#237; como en enfermedades infecciosas como el    HIV <sup>16</sup> y la hepatitis C <sup>17</sup>. En individuos sanos la prevalencia    se encuentra entre el 2-8 % aunque se incrementan con la edad<sup> 18</sup>.    </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> El objetivo de    este estudio fue determinar la prevalencia de los anticuerpos anti C1q en enfermos    con LES en la fase activa de la enfermedad. </font></p>     <p>&nbsp;</p>     <p> <font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b><font size="3">M&#201;TODOS</font></b>    </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Se estudiaron    pacientes con enfermedades autoinmunes provenientes de la consulta de Inmunolog&#237;a    del Instituto de Hematolog&#237;a e Inmunolog&#237;a y de los servicios de Reumatolog&#237;a,    Dermatolog&#237;a y Medicina Interna del Hospital General Docente "Enrique Cabrera",    atendidos en el per&#237;odo entre marzo de 2013 y marzo de 2015. Se dividieron    en dos grupos: LES en la fase activa seg&#250;n el &#237;ndice de actividad    del lupus eritematoso sist&#233;mico (SLEDAI), otras enfermedades reumatol&#243;gicas    autoinmunes tambi&#233;n en fase activa. Se incluy&#243; un grupo formado por    30 donantes sanos del Banco de Sangre. Las muestras se obtuvieron por punci&#243;n    venosa, se centrifugaron y se guardaron a - 20&#176;C hasta el momento de la    determinaci&#243;n. Los sueros hemolizados y lip&#233;micos fueron eliminados    por su posible interferencia. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Se determin&#243;    la concentraci&#243;n de anticuerpos anti C1q en el suero de los tres grupos    estudiados mediante un ensayo inmunoenzim&#225;tico (ELISA) cuantitativo de    la firma comercial (Orgectec) seg&#250;n las instrucciones previstas por el    fabricante. </font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Se utilizo un    test de Chi cuadrado para comparar la prevalencia de los anticuerpos anti C1q    en las diferentes enfermedades. No fue significativamente m&#225;s alta en los    pacientes con LES (40 %) que en los pacientes con otras enfermedades reumatol&#243;gicas    autoinmunes (20,5 %) y en individuos sanos (13,3 %). </font></p>     <p>&nbsp;</p>     <p> <font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b><font size="3">RESULTADOS</font></b>    <font size="3">&#160;&#160; </font></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Del total de enfermos    estudiados (n=64), 58 eran mujeres (90,6 %); 20 ten&#237;an LES en fase activa    seg&#250;n criterios SLEDAI y 44 otras enfermedades reumatol&#243;gicas autoinmunes.    </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Se encontr&#243;    presencia de anticuerpos anti C1q en 8 de los pacientes con LES en fase activa,    lo que representa el 40 % de positividad. En el grupo de enfermos con otras    enfermedades 9 resultaron positivos (20,5 %). Entre los donantes sanos 4 (13,3    %) presentaban presencia de anticuerpos anti C1q. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> La prevalencia    de los anticuerpos anti C1q. no fue significativamente m&#225;s alta en los    pacientes con LES que en los pacientes con otras enfermedades reumatol&#243;gicas    autoinmunes y en individuos sanos. </font></p>     <p>&nbsp;</p>     <p> <font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b><font size="3">DISCUSI&#211;N</font></b>    </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> De los pacientes    estudiados existi&#243; un predominio del sexo femenino, lo que concuerda con    lo reportado para estas enfermedades, en las que el factor hormonal pudiera    ser importante en el desencadenamiento. Resulta relevante el elevado porcentaje    de enfermos con anticuerpos anti C1q demostrados en el grupo con LES en fase    activa. Este hallazgo pudiera explicar la afectaci&#243;n de las funciones biol&#243;gicas    del C1q al depositarse una mayor cantidad de inmunocomplejos, provocando que    se amplifique el da&#241;o tisular que existe en esta enfermedad, con lo que    se empeora el fen&#243;meno inflamatorio <sup>19-21</sup>. Los trastornos en    la eliminaci&#243;n de los debris celulares apopt&#243;ticos, provocan un incremento    en la producci&#243;n de autoanticuerpos que contribuye a incrementar el da&#241;o    existente <sup>19-21</sup>. </font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> Es importante    contar dentro del panel diagn&#243;stico para enfermos con LES con biomarcadores    que permitan establecer el da&#241;o org&#225;nico y la respuesta al tratamiento    sin el riesgo de m&#233;todos invasivos como las biopsias u otros procederes    que impliquen el uso de radion&#250;clidos. </font></p>     ]]></body>
