<body><![CDATA[<p>&nbsp;</p> <hr>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>RESUMEN</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Fundamento: </b>la lesi&oacute;n del ligamento cruzado posterior es infrecuente, lo provocan los  traumas de alta energ&iacute;a y se acompa&ntilde;a de lesiones asociadas de partes blandas y  &oacute;seas.     <br>   <b>Objetivo:</b> profundizar los conocimientos en relaci&oacute;n a las lesiones  del ligamento cruzado posterior de la rodilla.     <br>   <b>M&eacute;todos:</b> se realiz&oacute; la b&uacute;squeda de la informaci&oacute;n desde  el primero de diciembre de 2016 hasta el 31 de enero de 2017 y se emplearon las  siguientes palabras: <i>posterior</i> <i>cruciate</i> <i>ligament</i> <i>injuries</i> y <i>posterior</i> <i>cruciate</i> <i>ligament</i> <i>reconstruction</i>,  a partir de la informaci&oacute;n obtenida se realiz&oacute; una revisi&oacute;n  bibliogr&aacute;fica de un total de 254 art&iacute;culos publicados en las bases de datos  PubMed, Hinari, SciELO y Medline mediante el gestor de b&uacute;squeda y administrador  de referencias EndNote, de ellos se utilizaron 51 citas seleccionadas para  realizar la revisi&oacute;n, todas ellas de los &uacute;ltimos tres a&ntilde;os, donde se incluyeron  tres libros.     <br>   <b>Desarrollo:</b> se abordan los factores  relacionados con la biomec&aacute;nica y anatom&iacute;a del ligamento cruzado posterior. Se  hace referencia al mecanismo de producci&oacute;n, diagn&oacute;stico basado en elementos  cl&iacute;nicos e imaginol&oacute;gicos. La indicaci&oacute;n quir&uacute;rgica es abordada de manera  precisa, as&iacute; como las diferentes t&eacute;cnicas. En relaci&oacute;n al tratamiento  quir&uacute;rgico de las lesiones combinadas se describe el tiempo de realizaci&oacute;n.  Para culminar se hace referencia a las complicaciones divididas en preoperatoria,  transoperatoria y posoperatorias.     <br>   <b>Conclusiones: </b>las lesiones del ligamento cruzado posterior son tratadas  de forma quir&uacute;rgica rara vez en caso de ser aisladas, no as&iacute;  en lesiones combinadas donde se requiere de reparaci&oacute;n.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>DeCS:</b> LESIONES DEL LIGAMENTO CRUZADO ANTERIOR; RODILLA; IMAGEN POR RESONANCIA  MAGN&Eacute;TICA; PROCEDIMIENTOS QUIR&Uacute;RGICOS OPERATIVOS; LITERATURA DE REVISI&Oacute;N COMO  ASUNTO.</font></p> <hr>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"></font><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>ABSTRACT </b></font></p>     <p align="justify"><font size="2"><b><font face="Verdana, Arial, Helvetica, sans-serif">Background:</font></b><font face="Verdana, Arial, Helvetica, sans-serif"> posterior cruciate  ligament lesions are uncommon and are generally caused by high energy trauma,  usually associated to soft tissue and bone lesions.     ]]></body>
<body><![CDATA[<br>   <b>Objective:</b> to deepen into  knowledge about lesions of the posterior cruciate ligament.     <br>   <b>Methods:</b> a search in the databases PubMed, Hinari, SciELO and Medline was done  through the information locator EndNote by using the following words: <i>posterior cruciate ligament</i> <i>injuries</i> and <i>posterior cruciate ligament reconstruction</i>, resulting in a total of  254 articles which 51 of them selected for review, all of them in the last  three years, including three books.     <br>   <b>Development:</b> important  aspects related to the anatomy of the posterior cruciate ligament and  biomechanics are pointed out. Mechanism of injury and diagnosis based on  clinical and imaging were described. Surgical indications were stated as well  as operative techniques. In regards to combined lesions surgical time was pointed  out to perform it. Surgical complications were divided by preoperative,  transoperative and postoperative ones. </font></font>    <br>   <font size="2"><font face="Verdana, Arial, Helvetica, sans-serif"><b>Conclusions:</b> isolated lesions of the posterior cruciate ligament are seldom treated  surgically, but combined lesions do, because reconstructive procedures are  needed. </font></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>DeCS:</b> ANTERIOR CRUCIATE LIGAMENT INJURIES;  KNEE; MAGNETIC RESONANCE IMAGING; SURGICAL PROCEDURES, OPERATIVE; REVIEW  LITERATURE AS TOPIC. </font></p> <hr>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b><font size="3">INTRODUCCI&Oacute;N</font></b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El ligamento cruzado posterior (LCP) es el m&aacute;s fuerte  de la articulaci&oacute;n de la rodilla y su lesi&oacute;n se puede observar de un 38 % a un  44 % de los traumas de la articulaci&oacute;n. Las lesiones aisladas del LCP son  infrecuentes y representan el 3,5 %, sin embargo las lesiones combinadas son  las m&aacute;s reportadas hasta en un 56 % de los casos. La combinaci&oacute;n de lesi&oacute;n de  LCP y ligamento cruzado anterior (LCA) es del 45,9 %, la inestabilidad postero-lateral  se asocia en el 41,2 %. <sup>1-3</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Las lesiones del LCP son provocadas por trauma de gran  energ&iacute;a, de all&iacute; que por lo general se asocia a un gran n&uacute;mero de lesiones y de  mayor complejidad. El diagn&oacute;stico cl&iacute;nico es muy importante, basado en la cinem&aacute;tica  del traumatismo, inspecci&oacute;n, palpaci&oacute;n, maniobras espec&iacute;ficas y evaluaci&oacute;n del  estado vasculo-nervioso de la extremidad afectada. <sup>4-6</sup>  Los ex&aacute;menes imaginol&oacute;gicos son de gran ayuda para el  diagn&oacute;stico y entre ellos est&aacute;: la radiograf&iacute;a simple, la de estr&eacute;s, la imagen  de resonancia magn&eacute;tica, que es el est&aacute;ndar de oro y la tomograf&iacute;a axial  computarizada. <sup>7-9</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El tratamiento de las lesiones del LCP depende si  existen o no lesiones asociadas, ya que en las aisladas solo el 3 % de los  enfermos necesita de tratamiento quir&uacute;rgico, debido a que la mayor&iacute;a responde de  manera adecuada al tratamiento conservador, incluso en atletas. Por otra parte,  en caso de lesiones asociadas, la conducta por lo general es quir&uacute;rgica en  todos los casos, con excepci&oacute;n de los no aptos para esta modalidad de  tratamiento. Las variantes de tratamiento son m&uacute;ltiples y pueden ser por v&iacute;a  artrosc&oacute;pica, abiertas o combinadas, para la reconstrucci&oacute;n se emplean  diferentes tipos de injertos. <sup>10-12</sup> </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Debido a la importancia de este tema en los traumas de  la articulaci&oacute;n de la rodilla y la ausencia de publicaciones provinciales y  nacionales, propuso realizar la revisi&oacute;n, basada en los aspectos m&aacute;s esenciales  de las lesiones del LCP. </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b><font size="3">M&Eacute;TODOS</font></b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La b&uacute;squeda de la informaci&oacute;n se realiz&oacute; en un  periodo desde el primero de diciembre de 2016 hasta el 31 de enero de 2017 y se  emplearon las siguientes palabras: <i>posterior  cruciate</i> <i>ligament injuries</i> y <i>posterior cruciate ligament reconstruction</i>, a partir de la informaci&oacute;n obtenida se realiz&oacute; una  revisi&oacute;n bibliogr&aacute;fica de un total de 254 art&iacute;culos publicados en las bases de  datos PubMed, Hinari, SciELO y Medline mediante el gestor de b&uacute;squeda y  administrador de referencias EndNote, de ellos se utilizaron 51 citas  seleccionadas para realizar la revisi&oacute;n, todas ellas de los &uacute;ltimos tres a&ntilde;os,  donde se incluyeron tres libros.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Se consideraron estudios de  pacientes con lesi&oacute;n del LCP y otras lesiones asociadas de la rodilla. Se  excluyeron las investigaciones de pacientes que abordaban el tratamiento del  LCP mediante osteotom&iacute;as y artroplastias. </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El LCP desempe&ntilde;a un papel muy importante en la  estabilidad de la articulaci&oacute;n de la rodilla, este ligamento lleva su nombre a  su lugar de inserci&oacute;n en la regi&oacute;n posterior de la tibia proximal. El sitio de  origen es en la parte interna del c&oacute;ndilo medial del f&eacute;mur en un &aacute;rea  semicircular y c&oacute;ncava cercana a la escotadura intercond&iacute;lea. La inserci&oacute;n en  la tibia ocurre detr&aacute;s de la espina tibial y por debajo de la l&iacute;nea articular. <sup>13-15</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La longitud promedio del LCP es de 38 mil&iacute;metros y de  ancho 11 mil&iacute;metros. La sustancia del ligamento est&aacute; conformada por dos  fasc&iacute;culos el antero-lateral y el postero-medial, el nombre de los fasc&iacute;culos  es dado por la posici&oacute;n en la huella del f&eacute;mur. La huella femoral est&aacute; ocupada  en un 55 % por el fasc&iacute;culo antero-lateral y el restante 45 % por el fasc&iacute;culo  postero-medial. El fasc&iacute;culo antero-lateral es el que aporta mayor fortaleza al  LCP. La inserci&oacute;n de los dos fasc&iacute;culos ocurre en la fosa del LCP en la tibia  proximal. <sup>16-18</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El mecanismo de producci&oacute;n de las lesiones del LCP  var&iacute;a desde la lesi&oacute;n aislada o combinada. El mecanismo cl&aacute;sico consiste en un  trauma directo sobre la tibia proximal con la rodilla en flexi&oacute;n, lo que puede  ser observado en accidentes automovil&iacute;sticos al chocar la pierna con la pizarra  del auto o en los deportes de contacto. Por otra parte, otro mecanismo consiste  en la hiper-flexi&oacute;n de la rodilla donde ocurre la lesi&oacute;n en la inserci&oacute;n tibial  o a trav&eacute;s de su sustancia, por lo general en esta modalidad el fasc&iacute;culo  antero-lateral esta da&ntilde;ado, no as&iacute; el postero-medial que se mantiene intacto.  Por &uacute;ltimo, la lesi&oacute;n del LCP tambi&eacute;n puede ser provocada por un mecanismo de  rotaci&oacute;n externa con la rodilla casi en extensi&oacute;n y sin apoyo del peso corporal  por la extremidad. Debido a que ocurren por lo general otras lesiones se pueden  combinar otros mecanismos en varo y valgo. <sup>19-21</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El diagn&oacute;stico de las lesiones del LCP est&aacute; basado en  los antecedentes del mecanismo de producci&oacute;n descritos con anterioridad, el  cuadro cl&iacute;nico y los ex&aacute;menes complementarios en especial los de tipo  imaginol&oacute;gicos. <sup>22-24</sup>  Debido a que en su mayor&iacute;a son lesiones provocadas por  trauma de alta energ&iacute;a, se debe buscar a la inspecci&oacute;n elementos que traduzcan  alg&uacute;n da&ntilde;o vascular, entre los que se encuentran signos de hemorragia activa,  isquemia distal de la extremidad, hematoma con r&aacute;pida expansi&oacute;n, color y llene  capilar. Se deben detectar estigmas de traumas directos como contusiones,  heridas, alineaci&oacute;n de la extremidad, as&iacute; como aumento de volumen. <sup>25-27</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La palpaci&oacute;n debe estar enfocada a corroborar el  estado vascular y neurol&oacute;gico de la extremidad, y detectar alguna lesi&oacute;n de las  estructuras capsulares, ligamentosas y &oacute;seas que presentan dolor e inflamaci&oacute;n.  La movilidad articular debe ser explorada, ya que la limitaci&oacute;n a la extensi&oacute;n  significa posible lesi&oacute;n de meniscos, de los m&uacute;sculos de la corva y del  mecanismo extensor, mientras que el dolor en la interl&iacute;nea articular en flexi&oacute;n  de m&aacute;s de 90 grados sugiere una afecci&oacute;n de menisco como lesi&oacute;n asociada. <sup>28-30</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Las maniobras m&aacute;s empleadas para corroborar el  diagn&oacute;stico de lesi&oacute;n del LCP y otras lesiones asociadas son: las pruebas de  caj&oacute;n posterior, estr&eacute;s en valgo, varo, prueba de Lachman y pruebas de caj&oacute;n  rotatoria, as&iacute; como la prueba de <i>recurvatum</i>.  <sup>31-32</sup>  Se tuvo en cuenta todos los elementos anteriores, las  inestabilidades de la rodilla relacionadas con la lesi&oacute;n del LCP pueden ser  clasificadas en tres tipos, tipo A: inestabilidad rotatoria axial aislada, tipo  B: inestabilidad rotatoria asociada a laxitud en varo o valgo hasta un punto  determinado, tipo C: inestabilidad rotatoria asociada a laxitud en varo o valgo  sin l&iacute;mite o sin punto determinado. <sup>33,34</sup> Seg&uacute;n Fanelli GC <sup>35</sup>  la tipo A es la m&aacute;s observada. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Dentro de los estudios imaginol&oacute;gicos la imagen de  resonancia magn&eacute;tica es el m&aacute;s empleado ante este tipo de lesiones, ya que  permite su identificaci&oacute;n en los procesos agudos, as&iacute; como la de lesiones  asociadas de cart&iacute;lagos, meniscos y otras estructuras ligamentosas. Sin  embrago, este examen no es muy &uacute;til en lesiones cr&oacute;nicas debido a la posible  cicatrizaci&oacute;n de la lesi&oacute;n y en este caso hay que apoyarse en el m&eacute;todo  cl&iacute;nico. Por otra parte, el ultrasonido din&aacute;mico ha demostrado ser efectivo  para confirmar el diagn&oacute;stico de lesi&oacute;n del LCP. <sup>36-38</sup>    <br>       <br> La indicaci&oacute;n quir&uacute;rgica en las lesiones aisladas el  LCP es limitada y considerada como controversial, de all&iacute; que las m&aacute;s  justificadas son: lesi&oacute;n del LCP asociada a otras lesiones ligamentosas, lesi&oacute;n  del LCP con afecci&oacute;n de cart&iacute;lagos y meniscos, fracturas avulsivas en la  inserci&oacute;n del ligamento y la lesi&oacute;n asilada del LCP con inestabilidad  funcional. <sup>39-41</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> En relaci&oacute;n a las t&eacute;cnicas quir&uacute;rgicas de  reconstrucci&oacute;n del LCP las m&aacute;s empleadas en la actualidad son: la incrustaci&oacute;n  tibial abierta, la artrosc&oacute;pica y la artrosc&oacute;pica trans-tibial, <sup>20</sup> cada  una con elementos a favor y otros en contra (<a href="#tabla1">tabla 1</a>). <sup>42-44</sup></font></p>     <p align="center"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <br> </font><img src="img/revistas/amc/v21n6/t01140617.jpg" alt="tabla 1" width="583" height="524" longdesc="../img no 6/5052/t01140617.jpg"><a name="tabla1"></a></p>     <p align="left"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La t&eacute;cnica trans-tibial por v&iacute;a artrosc&oacute;pica est&aacute;  indicada en pacientes con da&ntilde;o de m&uacute;ltiples ligamentos, fracturas avulsivas,  inestabilidad funcional cr&oacute;nica y fallo del tratamiento conservador. <sup>45-47</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La selecci&oacute;n de la t&eacute;cnica quir&uacute;rgica depende de la  preferencia del cirujano, las caracter&iacute;sticas de cada paciente y las  condiciones materiales. Los mejores resultados de la cirug&iacute;a reconstructiva son  obtenidos entre la segunda y tercera semana de la lesi&oacute;n. <sup>48,49</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Con respecto al tipo de injerto a utilizar, existe  controversia y la selecci&oacute;n del injerto para la reconstrucci&oacute;n del ligamento  cruzado anterior (LCA) difiere del LCP ya que son estructuras biomec&aacute;nicas  diferentes. Sin embargo, est&aacute; justificado el uso de auto-injertos como el tend&oacute;n  rotuliano, semitendinoso y semimembranoso y el tend&oacute;n del cu&aacute;driceps. Por otra  parte, el uso de alo-injerto en especial el de tend&oacute;n Aquileano es el m&aacute;s  empleado para la reconstrucci&oacute;n del LCP en un 43 % en las lesiones agudas y en  un 50 % en las cr&oacute;nicas, y es el preferido por su longitud. Adem&aacute;s del tend&oacute;n  de Achiles, otros alo-injertos pueden ser empleados como el tend&oacute;n rotuliano, los  tendones semitendinoso y semimembranoso, y tend&oacute;n del cu&aacute;driceps. <sup>22,25,30</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La reconstrucci&oacute;n puede ser realizada a simple o doble  fasc&iacute;culo, seg&uacute;n la necesidad del enfermo y adiestramiento del equipo m&eacute;dico. <sup>41,50</sup>    <br>   Los procedimientos reconstructivos son realizados  sobre el LCP, el LCA y las esquinas postero-mediales y postero-laterales, para  las cuales existen t&eacute;cnicas espec&iacute;ficas. El tiempo de reparaci&oacute;n de la afecci&oacute;n  combinada de LCP, LCA y lado medial est&aacute; en dependencia de la clasificaci&oacute;n de  las lesiones del lado medial. La gran mayor&iacute;a de las lesiones mediales  cicatrizan en un periodo de cuatro a seis semanas y pueden ser tratadas con  ortesis. Las lesiones tipo A y B son tratadas de forma simult&aacute;nea en un solo  tiempo quir&uacute;rgico, no as&iacute; las tipo C en las que la reparaci&oacute;n medial o  postero-medial es realizada en las primeras dos semanas y luego en otro tiempo  quir&uacute;rgico de tres a seis semanas se reparan los LCP y LCA. <sup>11,16,20</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Las lesiones combinadas del LCP, LCA y esquina  postero-lateral tambi&eacute;n son tratadas seg&uacute;n la clasificaci&oacute;n de las lesiones  laterales, en las lesiones tipo A y B son tratadas en un solo tiempo  quir&uacute;rgico, sin embargo en caso de lesiones tipo C, la reconstrucci&oacute;n lateral o  postero-lateral se realiza en la primera semana y las del LCP y LCA entre tres  y seis semanas. <sup>12,16,24</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Existen un grupo de complicaciones relacionas con el  trauma en s&iacute; y otras relacionadas con el tratamiento empleado, en especial el  quir&uacute;rgico, la incidencia de complicaciones en pacientes con lesi&oacute;n del LCP es  reportada hasta en un 44 % en los traumas agudos de la rodilla. <sup>27,28</sup>  En el trauma inicial la mayor incidencia de  complicaciones se reportan en la afecci&oacute;n neuro-vascular, ya que el da&ntilde;o  arterial por desgarro de la capa intima arterial y secci&oacute;n completa que  requiere de cirug&iacute;a vascular es del 32 % como promedio con un rango del 16 % al  64 %, seg&uacute;n plantea Marcus MS, et al. <sup>51</sup> Por otra parte, la lesi&oacute;n  nerviosa m&aacute;s encontrada es la del nervio peroneo com&uacute;n entre un 10 % y 40 %. <sup>50,51</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Las complicaciones trans-operatorias reportadas con  m&aacute;s frecuencia son: el da&ntilde;o neuro-vascular, la osteonecrosis del c&oacute;ndilo  femoral medial, la fractura de r&oacute;tula o platillo tibial, el s&iacute;ndrome  compartimental y las relacionadas con el uso de torniquete. Sin embargo, las m&aacute;s  frecuentes son las neuro-vasculares por la proximidad de la arteria y nervio  popl&iacute;teo cercanos al cuerno posterior del menisco lateral, que oscila a una  distancia de siete a ocho mil&iacute;metros.<sup>50,51</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> En relaci&oacute;n a las complicaciones posoperatorias las  m&aacute;s encontradas son: la persistencia de la laxitud despu&eacute;s de la reparaci&oacute;n,  artrofibrosis, dolor anterior de la rodilla, osificaci&oacute;n heret&oacute;pica e  infecci&oacute;n. <sup>47,51</sup></font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Las lesiones del LCP no son lesiones frecuentes, sin  embargo su presencia traduce la ocurrencia de un trauma de alta energ&iacute;a que  afecta las estructuras &oacute;seas y de partes blandas de la articulaci&oacute;n de la  rodilla.
