In order to update the knowledge about the characteristics of hypertension in the elderly, some recent concepts connected with its prevalence, physiopathology and mode of control were reviewed. Emphasis was made on the importance of isolated systolic hypertension as a form of frequent presentation and on its relationship with the pathogeny of left ventricular hypertrophy and of renal failure. The biochemical modifications and of the vascular arbor accompanying aging were approached. The advantages obtained with the use of low doses of diuretics and betablockers on the cerebrovascular and cardiovascular mortality were commented on. It was concluded that the hipertensive elderly should have a rational strategy in his attention including the gradual decrease of the elevated readings of arterial pressure and the selection of drugs with the lowest level of side effects so as to guarantes an optimal quality of life.
