The case of a patient with hemicranial headache that became very persistent, serous nasal secretion, ocular manifestations, fever, focal signs and, later, with manifestations of vasculitis in the upper and lower limbs, is presented. The anti-neutrophile cytoplasmatic antibodies (cANCA) with values of 1:160 were investigated as well as the biopsy of the nasal mucosa, where necrotizing vasculitis was found. Treatment with cyclophosphamide and prednisone was indicated. The total remission and the dissapearance of the titres of cANCA was attained in 8 months. A correlation between the clinical improvement and the cANCA values was observed in our patient
