Transfusion therapy is a science of ongoing development. At present, the advantages of the individual blood component transfusions have restricted the use of whole blood. The clinical parameters rather than hemoglobin figures are determining in deciding transfusion of red cell concentrates. It has been proved that refractoriness to platelet administration as a consequence of alloimmunization is high, so indications for their use should be clear. The low effectiveness of granulocyte concentrates and their multiple adverse effects have questioned their use. Special cellular components are expensive and require embarrassing procedures for their manufacture and therefore, they should be used in selected patients under strict indications. The anatomical and physiological characteristics of the pediatric patients specially infants and newborn provide transfusion therapy with certain particularities. The benefits of transfusion of cellular components are real; however, important adverse effects may come up and that is why each indication demands a thorough assessment that assures a better use and the success of hemotherapy
