A patient of 55 years old was presented. He suffered an acute myocardial infarction of inferior face with extension to right ventricle Killip-Kimbal IV with an interventricular communication and a great hemodinamic impairment to which a Swan-Ganz catheter was placed. It allowed not only the corroboration of diagnosis but also it was useful for the adequate management o fluids and pressure amines which made possible patient stabilization for his posterior surgical restoration.
