Abstract

A case of a patient presenting with chronic renal failure admitted to undergo a renal transplantation for suffering an acute respiratory failure resulting from a big arterial hematoma as a complication of a deep venous catheterization, is reported. During the performance of an emergency tracheostomy an accidental total section of the right carotid artery was produced which at the moment of the surgical incision was found to be in the medial line due to a displacement of the neck structures by the expanding hematoma. The placement of a number 12 aspiration catheter because tracheal intubation was not possible, allowed to maintain an adequate oxigenation during the placement of the tracheostomy catheter. The proper circulation through the contralateral arterial vesels allowed to keep a good brain perfusion and at the end of the vascular repair the patient was free of neurologic sequelae
