Abstract

The aim of this study was to know the immediate results obtained with laparoscopic cholecystectomy in patients over 70. 3 900 patients, of whom 329 (8.4 %) were over 70, were operated on during the established period. 7l.9 % were females. 276 patients (83.9 %) were between 70 and 79 years old. 50 (15.2 %) were included in the group aged 80-89 and 3 had lived 90 or more years. 76 % suffered from some associated disease with predominance of the cardiovascular diseases. 58 surgical accidents (17.7 %) occured, 2 were due to injuries of the main bile duct (0.6 %). The rest were caused by opening of the gall bladder with falling of calculus in the cavity (12 cases) or without it (44). There were 12 conversions (3.5 %). The main causes were inflammatory processes and duodenal fistula in 4 and 2 patients, respectively. 11 patients (3.3 %) had some complication and 5 were considered as severe: 2 injuries of the main bile duct, l generalized peritonitis, 1 subphrenic hematoma and 1 subhepatic biliary collection. 2 of them were treated by videolaparoscopy. The case of a ninety-year-old woman who suffered from an injury of the main bile duct that was not diagnosed at the time of the operation was the only death reported (0.3 %). It was considered that minimally invasive surgery is a safe option for the elderly with vesicular lithiasis, even in patients at high risk for associated diseases
