Abstract

The presence of an esophagopleurocutaneous fistula is a complex problem. A case of difficult solution due to the oncological history and the existance of an esophagopleurocutaneous fistula is reported. A male patient with history of right pneumonectomy due to lung cancer 10 years before, with thoracic fistula of a year of evolution and loss of 16.36 kg (36 pounds) of weight, that was satisfactorily treated with an exclusion technique of the fistulous process by retrosternal tubular esophagogastroplasty with cervical anastomosis (Postleitwait's technique) is presented. On clousering the esophagus, the fistula was left by exclusion in the cervical region distal to anastomosis. The fistula closed completely at 2 weeks and the patient was discharged with the digestive tract restored. The patient's hemithorax was frozen by pneumonectomy, which impeded the direct surgery. The exclusion was the only possible method. The retrosternal tubular esophagogastroplasty solved a complex problem.
