Abstract

INTRODUCTION: From introduction of laparoscopic cholecystectomy, method of 4 entrances is standard procedure. To reduce surgical invasion techniques of mini-laparoscopic cholescystectomies has been described including those with vesicular traction with ligatures. Present study assess effectiveness and safety of two new laparoscopic instruments developed and patented by author, which allow us to perform a new modality of mini-laparoscopic cholecystectomy from two trocars, combined with vesicular traction with ligatures. RESULTS: Feasibility and safety were statistically comparable for both groups (p = 0, 1340; p = 1, 00). For patients of experiment group, average surgical time was greater (59, 04 min versus 37, 26 min; p = 0.000); however, cosmetic result was higher (wound assessment of 1,23 versus 2,53; p = 0.00). CONCLUSIONS: Vesicular retractor and knot sleeping with simultaneous coaxial traction allow us to perform a variant of mini-laparoscopic cholecystectomy by means of thread traction, advantaging preceding thread traction techniques. Technique requires experience in laparoscopic surgery. Effectiveness and safety as well as aesthetic advantages were evidently demonstrated.
