We performed a longitudinal, retrospective, clinico-anatomopathologic study in 192 patients, deceased in "Abel Santamaría" Clinical Surgical Hospital in Pinar del Río province, with a diagnosis of acute myocardial infarct (AMI) from January 1994 to December 1996. The aim ot thist study was try to determine which are major causes hampering diagnosis in lifetime . In 33.7 % of patients diagnosis wans't made in lifetime, and of these, 63.8 % were elderly over 70 years old, in 69.4 % was impossible to diagnose whereas, because of technical faults in 15.3 % it was impossible to made, and in a similar percentage, acute myocardial infarction only was a contributing cause to death in these sick persons. Complications and commonest diseases associates with infarctions, were heart failure (33.3 %), encephalic vascular accident and bronchopneumonia (20.8 %), respectively, sudden death (11.0 %) and choronic renal failure (8.3 %). Authors conclude that in old age, diagnosis of AMI it is not easy to made, since usually presenting with an atypical symptomatology, angina sine dolore or masked because of its own complications of symptoms of other diseases
