Surgery as an initial treatment tries to remove radiotherapy from the place it has had historically. In order to analyze the exact indications of every therapeutic modality based on prognostic factors, a bibliographic review of the present status of this polemic is analyzed, making emphasis on variables such as: the clinical form, the invasion of the anterior commissure, the subglottic invasion, the mobility and the important reduction of the glottic space. It is concluded that studies are needed to give an answer to many of the questions that have arised about this problem.
