The multisite pacing is a therapeutic alternative for patients with severe ventricular dysfunction. This technique was used in 6 patients with functional class III-IV refractory heart failure and left bundle branch block to evaluate its efficiency. Variable PR intervals were programmed for biventricular pacing between the output tract and the apex of the right ventricle. A nuclear ventriculography in basal state was performed on the 7th day and on the 6th month after the implantation to measure the ejection fractions, the ventricular synchronism and the mitral regurgitation index. During the follow-up, 2 patients had refractory heart failure and one of them died (bifocal pacing). The improvement of the ejection fraction and of the functional capacity were evident with the biventricular pacing. This technique proved to be efficient in the treatment of severe ventricular dysfunction.
