Abstract

An analytical, prospective and longitudinal study was conducted by following up the neurodevelopment during the first 2 years of corrected age of 86 newborn infants with very low birth weight that were discharged from "Ramon Gonzalez Coro" Gynecoobstetric Hospital between December, l988, and August, 1993. The objective of this study was to evaluate the neurodevelopment in ventilated low birth weight newborn infants. They were divided into 2 groups: a) Ventilated (26 patients) and b) Non-ventilated (60 patients). There were no differences between them as regards sex, weight and gestational age. The highest values of Apgar score were observed in the ventilated newborn infants. No differences were found between both groups in relation to the evolution of neurodevelopment, but it was detected a higher number of severe neurological sequelae in the group of the ventilated in comparison with the non-ventilated (l9.2 vs 1.7%). The most frequent mild alterations of neurodevelopment found in the ventilated newborn infants were: hyperactivity, reflex alterations, transient disorders of the muscular tone and mild speech retardation. Spastic cerebral palsy was the commonest sequela (11.5%) within the severe alterations. It was concluded that mechanical ventilation as such does not seem to be associated with a higher index of sequelae of the neurodevelopment in newborn infants with very low birth weight, although it does seem to act as an aggravating factor of the already existing lesions in those cases with severe depression at birth.
