Acidosis is a manifestation of metabolic disorders in the organism that may reflect hypovolemia, hypoxia, sepsis and utilization of alternative metabolism in the production of energy. Its early diagnosis, and mainly the prevention in the critically ill patient condition its evolution. There are various classifications that try to show the metabolic and hemodynamic state of the patient at the intensive care services; however, the problem appears at the moment of giving attention to a patient. To restore volume, to nourish, to oxygenate, or to use bicarbonate could be therapeutic options. The problem is when and how to do it. This review is made aimed at updating our physicians as regards knowledge, diagnosis and attention to metabolic acidosis in sepsis
