Abstract

With the objective of evaluating the effects of ventilation in prono position in adults with acute pulmonary injure and of hemodynamics during ventilation, the effects achieved on ventilatory mechanism and oxygenation were compared with the values obtained in supine position. A study was made on all patients who were admitted to the Intensive Care Unit for Adults at "Dr Luis Díaz Soto" Higher Institute of Military Medicine, diagnosed with acute pulmonary injure and placed in prone position for 2 hours a day after sedation and hemodynamic stabilization. Arterial blood gas was determined before changing position at 30 min and at 2h, with continuous monitoring by pulse oximetry. PaO2, arterial blood oxygen saturation, alveolar-arterial oxygen difference (DAaO2) and dynamic compliance in one position or the other were evaluated and compared. When concluding this study, there was a rise of systolic blood pressure in ventilation in prone position. No hemodynamic complications attributable to this procedure occurred, and the ventilatory mechanism improved in prone position at one hour and at 2h. When comparing the values obtained in the supine position, an increase of PaO2, PaO2/FiO2 ratio and oxygen saturation was detected in ventilation in prone position with respect to supine position. However, PACO2 and DAaO2 values lowered.
