Abstract

A longitudinal, corelational and descriptive study of 43 AIDS patients from Cienfuegos and Sancti Spiritus, who were treated with highly active antiretroviral therapy, was performed. The response to treatment with domestically-made pharnmaceuticals was evaluated according to clinical and immunological variables. The follow-up lasted from May 2001 to June 2003. The studied cases were young male patients fundamentally, who had caught the infection by sexual intercourse. They developed the natural evolution of HIV infection since the average length of time from the diagnosis to AIDS definition exceeded 6 years and at the same time, most of the cases presented with extreme inmunodepresssion at the beginning of the therapy. Acute retroviral syndrome in its severe form as well as minor and major opportunistic infections were evident in a sizeable number of patients. These variables did not correlate with the immunological response of the patients. The highly active antiretroviral therapy had an impact on the hospital stay the major opportunistic infection indexes, which were reduced to zero; there was no deaths at the end of the study. It was observed that patients showed a sustained immunological recovery since the first year of therapy application. Adherence to treatment correlated with recovery of the immunological function of the patients and, it is significant that in the cases that did not adhere to treatment, no rise in TCD4+ lymphocytes count was documented. There was drug toxicity in over 90% of cases, with predominance of digestive disorders; the clinical findings suggestive of metabolic syndrome were found in 18.6% of cases, a frequency rate lower than that reported in the international series.
