Abstract

ENAMORADO ANAYA, Antonio Rafael et al. Two-year application of thrombolytic therapy in patients with acute myocardial infarction. Medicentro Electrónica [online]. 2022, vol.26, n.1, pp. 29-43.  Epub Jan 01, 2021. ISSN 1029-3043.

Introduction:

thrombolytic strategy not only improves survival expectancy after infarction, but also reduces overall one-month mortality in patients receiving this treatment.

Objective:

to characterize patients with ST-segment elevation acute myocardial infarction treated with recombinant streptokinase at “Celia Sánchez Manduley” Clinical and Surgical Teaching Hospital.

Methods:

a descriptive, longitudinal and retrospective study was conducted in 94 patients who received thrombolytic treatment from December 2018 to November 2020. Age, gender, comorbidities, time-related therapeutic window, causes of temporary suspension, complications, topography, clinical, electrical and enzymatic reperfusion criteria as well as mortality were the variables used in this study. Absolute and relative frequency and arithmetic mean with standard deviation were calculated.

Results:

male gender and group aged 61-70 years predominated. Patients with a history of hypertension and diabetes predominated. Arterial hypotension was the most frequent cause of thrombolysis discontinuation. The 69.1 % were thrombolyzed after 6 hours. Only 27 patients suffered complications; atrioventricular block and Killip-Kimball II were the most frequently seen complications. On the other hand, 9 patients did not show reperfusion criteria and only 7.4 % died.

Conclusions:

thrombolysis is performed in a minority of patients even in the therapeutic window period. Hypotension was the most frequent cause of temporary suspension of thrombolysis, and case fatality was lower than the overall case fatality for acute myocardial infarction.

Keywords : thrombolytic therapy; ST elevation myocardial infarction/therapy.
