Abstract

We present a 37 year-old man with no antecedents of abdominal pain, or suffering from other illnesses who received a direct blow with a blunt object (baseball bat) in the right lower quadrant of the abdomen. The patient was seen in consultation with a selective abdominal pain in his right iliac fossa, with approximately 29 hours of evolution that began after a closed abdominal trauma. A positive Blumberg sign, a painful McBurney’s point with cutaneous hyperesthesia and certain grade of muscular defense were verified on physical exam. Leukocytosis and prevalence of neutrophils with left deviation were proven on complementary exams. An exploratory laparotomy was decided and a phlegmonous acute appendicitis was verified during the periprocedural period. Traumatic etiology in acute appendicitis is a diagnosis for exclusion that should be considered under specific conditions.
