Ankylosing spondylitis is defined as chronic, inflammatory and idiopathic spondyloarthropathy, which generally begins at an early age. It is stated that it is predisposed by the male sex and its main clinical manifestation is the presence of a pain of inflammatory type located in the lower back and that mainly affects the lumbar spine. However, it can affect other joints such as the knees, shoulders and coxofemoral joints. The avascular necrosis of the head of the femur is the result of abrupt interruption of the blood supply to the bone of intrinsic or extrinsic causes, causing necrosis and bone collapse. We present the case of a young, male patient diagnosed with ankylosing spondylitis who presents with clinical and radiological manifestations that allow the diagnosis of avascular necrosis of the femoral head.
