The Morgagni diaphragmatic hernia usually occurs with a low frequent in adults due to its congenital origin. Therefore, it is often mistaken, imaginologically speaking, for a mediastinal lipoma, as in this case of interest. A case is presented of an adult with a Morgagni diaphragmatic hernia, which was surgically intervened by video-assisted thoracoscopy with a satisfactory postoperative evolution.
