A program for the care of the hypertensive elderly candidate to elective major surgery was evaluated. Clinical assessment, research and follow-up to the immediate postoperative were included in the evaluation. Once the existance of cardiovascular sympathetic disautonomy was discarded, it was used the cold pressor test as a physical test to produce arterial hypertension. This is an auxiliary method in the diagnosis of steady blood pressure control and a guide in therapeutical care. None of the 1 010 patients under study was suspended. It was concluded that the implementation of the program allows to attain a considerable reduction of the surgical suspensions due to arterial hypertension in the elderly.
