The case of a pregnant woman with 35.5 weeks of gestation with a ovarian giant cyst was presented. A distocic delivery by cesarean was produced and an ovarian giant cyst of 3 400 g was extracted, as well as a male newborn of 1 600 g with Apgar score 8/9. Both evolved successfully. The histological diagnosis of the ovarian tumor was concluded as a benign cystic teratoma associated with a mucinous cystoadenoma with follicular cysts with luteinization of techal cells.
