Abstract

A randomized, single, open placebo-uncontrolled clinical trial was conducted in a group of 20 severe patients that were admitted at the Adults' Intensive Care Unit of "Dr. Luis Díaz Soto" Central Military Hospital from October 2003 to June 2004 aimed at determining the behavior of some parameters related to the nutritional state on applying two calory-protein regimes in a group of selected patients. The main results were: 90 % of the total of the studied patients presented a reduction of the body weight percentage at the end of the treatment, and 10 % (one case) showed a discrete increase of it. There were no significant differences in the incidence of the major complications in both study groups. They were arranged according to their decreasing frequency as follows: hydroelectrolytic and acid-base disorders, severe sepsis, acute respiratory distress syndrome and septic shock. Mortality was similar in both study groups during the follow-up. A case was reported for each of the treatment schemes used. The above evidences that the critically ill or hypercatabolic patient has very specific nutritional requirements and not necessarily supraaugmented demands, so the nutritional support of the severe patients should be tailored to them, that is, adjusted to the individual requirements of the patient based on the changes observed daily.
