Abstract

SANSO SOBERATS, Félix José; VALDIVIA ONEGA, Cristina  and  LORENZO RODRIGUEZ, Alcidez. Propuesta de modelo para la transferencia de pacientes en el sistema de salud cubano. Rev Cubana Med Gen Integr [online]. 2003, vol.19, n.1, pp. 0-0. ISSN 1561-3038.

Since 1996, it was instructed that the individual medical histories used in the primary health care ceased to be ambulatory. From that date on, there is no official form for the patient transfer within the health care system. This project is the result of a research work and of the design of a model for referral and counter-referral of patients, as a linking between the professionals at various levels and the professionals at the primary health care centers. Prior to the design phase, the former individual ambulatory medical history, the hospital discharge form and the case referral model were examined and besides that, the outcome of previous research works about the transfer system and of a former model project for referral and counter- referral were taken into consideration. The new model proposal is aimed at fulfilling the following principles: it should be a unique, practical and comprehensive model to be carried by the patients during transfer or by the health system itself in those cases that require so. The advantage is that it allows us to standardize the information on ordinary patients to be exchanged, and theoretically speaking, it should contribute to improve the quality of the continued care to be given to the patients in our country. This paper presents the model and recommends its general use and also the control of the compliance with the provisions related to the referral and counter-referral system.
