This paper shows the experience accumulated in the implementation of a primary health care system under neoliberal conditions in a native population from La Guajira department in Colombia from 1997 to 1999. It sets forth the alternatives submitted to the health division of the department, with a family medicine approach, and applied to a native population under the local political conditions and some considerations are also presented in this regard. The permanent care given by a trained staff in distant areas guarantees the success of this experience.
