A 68-year-old female patient is referred to the department for an abdominal ultrasound, since she was suffering from dispeptic disorders and, on occasions, pain at the level of the right hypochondrium. Her physical examination was negative and the diagnostic impression of her physician was vesicular lithiasis. During the ultrasonographic examination, it was observed as a positive datum an aneurismatic dilation of the portal vein at the beginning of its intrahepatic path of 18 mm of diameter. The rest of the examination was negative. As it is an unusual case, it was decided to present it.
