The infrared videopupillography is a new technique to study the pupillary dynamics that is very useful in the diagnosis and evolution of diabetic autonomic pupillary diabetes (DAPD). We studied a group of diabetic patients and a sound control group by using this technique in order to determine its usefulness in the diagnosis of DAPD. A total of 40 healthy subjects and 40 type 1 diabetics with an average age of 34.7 and 49, respectively, were examined. The pupillary area in the darkness was 11.35 mm2 in the control group and 10.8 in the diabetic (the pupillary reflex latency in the diabetic was 0.95 s and 0.81 s in the control group) (p<0.05). The mean pupillary contraction speed (MCS) was 3.73 mm2/s in the control group and 3.42 in the diabetic patients (p> 0.05). The mean dilatation speed (MDS) was 3.23 mm2/s in the first group and 3.15 mm2/s in the second (p <0.05). It is concluded that through this technique it is possible to determine the sympathetic denervation of the pupil, based on the decrease of the pupillary area in the darkness and of the MDS, with a relative conservation of the parasympathetic system at the pupillary level due to the mild affectation of the MCS. It confirms the presence of DAPD in its initial stage of sympathetic affectation.
