Abstract

Objectives To substantiate and to implement the setting up of a municipal community clinic to take care of high obstetric risk patients, this will support the prevention of low birthweight and reduction of morbidity and mortality in this risk groups in the primary health care setting in Holguín. Methods The structural and functional design of the above mentioned clinic was prepared as well as the administrative and scientific-technical procedures to be developed in this center and the interrelationship with the medical assistance structures in the territory. The project was validated in the last three months of 2005 and was developed from 2006 to 2008 with a group of 800 pregnant women. Results There were found high incidence of patients who suffer great depression, anxiety and pathological stress, predominance of older maternal age related to low birthweight newborns, high incidence of newborns with insufficient weight (not low birthweighted) and high incidence of ultrasound-diagnosed cervical incompetence. The most frequent risk factors were anemia, malnutrition, pathological weight curve, cervical changes and unfavourable obstetric history. Conclusions The management of pregnant women admitted in the clinic, who gave birth in the study period, allowed validating the structural and functional design of the institution and certifying the administrative and scientific-technical procedures put into practice during the period of admission.
