Introduction: the participation of physicians in international cooperation missions required readjustments in primary health care. One of the modifications introduced was that local nursing personnel would be in charge of health care at educational institutions. Objective: evaluate the identification of health risk factors in school environments conducted by a primary health care nurse. Methods: a descriptive cross-sectional study was conducted from August 2008 to January 2009. Selection of the consultation office was based on the opinion of experts. The study universe was the population in the 5-14 age group (51 individuals) from Consultation Office 9 of Pueblo Nuevo people's council, municipality of Centro Habana. The nurse applied a previously validated observation guide. Three elementary schools and one junior high school were studied. Only classes with children and adolescents from the study consultation office (31 in all) were included. Results: it was found that lighting and microclimate conditions were deficient, the color of blackboards inadequate and the teaching environment noisy. Conclusions: the nurse accurately identified the health risk factors present in the school environments studied. The environmental diagnoses achieved by the nurse were confirmed by measurements made by specialist personnel, corroborating the quality of the diagnoses.
