Background: Fallot's tetralogy is a frequent cyanogenic congenital heart disease in childhood, which surgical treatment presented a successful rate in short and long terms. The history of "Blue disease" is another great step in the surgery of the heart . Objective: To highlight the works of the first surgeons that performed this procedure in the country. Method: Upon the basis of a documentary-historical research. Results: A case report: A 5 years old and 6 months boy, Pedro Cesar Pared y Mendoza, suffering from Fallot's tetralogy that was corrected at the described age in September 10, 1947, 2 years and 10 months after the first surgery in the world, the boy underwent the operation at "Cardona" Clinic, Havana, by the distinguished Cuban surgeon Dr. Antonio Rodriguez Diaz, being this dangerous surgery the first performed in Cuba. Conclusion: The facing of this kind of surgery, in that historical moment, represented a glory for Cuba and the medical professionals that are included on the list of the" Paradimgs of the Cuban Medicine".
