Testicle cancer constitutes 1% of the neoplasms in males; it is the most common tumor in men between 15 and 35 years old, most of them malignant. This paper was aimed at presenting a case of unilateral germinal testicular tumor of an uncommon clinical presentation that was diagnosed by ultrasound in a 37 years old patient, without personal neither pathological nor urologic history of interest, who was referred by the urologist to study his infertility. The patient presented no fever, pain, associated urological symptoms, and his general health status was not either affected. At physical examination a renitent palpable tumor in the right testicle was found. Left testicle showed a normal size and of augmented consistence, very discreetly painful, the epididymis had normal characteristics. Throughout the ultrasonographic study (suspecting a right varicocele), a testicular synchronic contralateral tumor (left) of a big size was visualized in the testicle showing a heterogeneous hypoechogenic pattern. Left inguinal orchiectomy was performed and in posterior pathologic studies the presence of unilateral testicular tumor having a histological component seminoma-type was confirmed.
