Introduction: higher education should ensure professionals with a solid theoretical and practical training according to the social needs. Objective: to support the role of the universities in promoting health and self-care in primary care. Material and methods: a literature review was conducted in search of theoretical, methodological and historical elements that trace out the route to active the universities in relation to health promotion and self-care. Analysis-synthesis was the method used. Results: this literature review showed that universities play a leading role and for the 21st Century the strategy of instruction and development of human resources is more relevant in accordance with the needs of health and the promotion of primary health care. The university can be a health promoter if this is the goal; having the potential to positively influence on life and health status of its members, protecting and promoting welfare, these higher education centers may lead and support the changes of the processes in the outside community, in accordance with policies and practices. Health self-care should become part of the professional routine. Conclusions: the universities play a leading role; having the potential to influence on the adoption of healthy lifestyles. Graduates may be able to encourage changes for the welfare and the health of institutions and communities.
