The syndrome of acute respiratory distress is a disease of low incidence but of high mortality, which is determined by its secundary organic function of a hypoxic refractory and maintained phenomenon. Pulmonary and extra-pulmonary clinical presentations are the most outstanding. Finding an effective treatment is a constant concern, with regards to this, the main bases are valid, among them modifying risk factors, identifying and treating the responsible disease, maintaining internal means (volume, electrolytes, acid-basic balance and nutrition) at optimum levels, the use of artificial ventilation with all its protecting strategies, hemodynamic stabilization and detailed management of inotropic drugs. It is vital to highlight the importance of medical and paramedical personnel training for identifying the symptoms and signs on the basis of the disease aimed at their early management and avoiding the progression of hypoxemia. This review pretends to describe and update the evolution of concepts about this syndrome, particularly in the pediatric population.
