Biopharmaceutical Research, Development and Innovation (R&D&i) is characterized by a low probability of success, high time and costs for its introduction to the market in highly regulated environments with high standards, constituting a challenge to be overcome by companies in the sector, which requires the adoption of integrated approaches to innovation management that meet the needs of this industry. The article aims to explain the characteristics of biopharmaceutical management at the international level and in Cuba, highlighting its complexity, the challenges and various lessons learned in the management of Cuban biopharmaceutical research, development and innovation (R&D&I). For this, the results achieved in some of the innovation projects of the main directors of the BioCubaFarma business group are presented during their graduation at the Higher School of State and Government Cadres. In particular, organizational innovations at the Center for Molecular Immunology are shown as a high-tech company case study. The method followed for the diagnosis, the use of prospective and innovation surveys, as well as in the generation of innovation projects can be generalized to other business systems in the country.
