Enfoque desarrollador en la enseñanza del diagnóstico clínico de anomalías dentomaxilofaciales

 

Developing approach in the teaching of clinical diagnosis of dentomaxillofacial anomalies

En la práctica educativa cubana actual las condiciones del proceso de enseñanza-aprendizaje en la asignatura de Ortodoncia en la carrera de Estomatología necesita que la enseñanza del diagnóstico clínico de anomalías dentomaxilofaciales se transforme para convertirse en sistémica, sistemática, flexible, personológica y contextual. Esta transformación debe revertirse e n la organización, ejecución y control de los contenidos conceptuales, procedimentales y actitudinales, para que los estudiantes logren identificar e interpretar, mediante el método clínico, la alteración morfológica, estética y funcional de las estructuras dentomaxilofaciales.

Para pasar a la enseñanza de este contenido con enfoque desarrollador, no sólo se debe atender la trasmisión de conocimientos sino considerar la unidad que en la actividad práctica tienen lo cognitivo instrumental, lo afectivo valorativo y lo conductual. El carácter sistémico de la enseñanza se logra al relacionar conocimientos, habilidades, hábitos, emociones, sentimientos, motivos, intereses y procesos metacognitivos, de modo que contribuya tanto a la instrucción como a la formación, a la educación y por consiguiente al desarrollo de los estudiantes.

El carácter personológico se logra al considerar el diagnóstico pedagógico integral desde la planificación de la consulta para la asignación de pacientes a los estudiantes, de manera que se aumente de forma gradual y progresiva la complejidad de las anomalías (síndromes de clase I, II y III; dentición temporal, mixta y permanente), la interpretación de los exámenes complementarios y el grado de cooperación de los pacientes y sus familiares. Se deben realizar acciones diferenciadas, que permitan el tránsito de la zona de desarrollo actual a la potencial. El diagnóstico pedagógico se actualizará de forma sistemática según la transformación que se logre en los estudiantes y el grupo.

Con el objetivo de lograr el carácter sistemático de esta enseñanza, las acciones se deben realizar desde el inicio de la asignatura. El contenido del crecimiento y desarrollo craneofacial, así como el de los dientes y la oclusión, pueden ser utilizados para enseñar a interrogar y a examinar. Se debe continuar la enseñanza del resto del contenido, a partir de la demostración del profesor y la ejercitación continua del estudiante con nuevos pacientes, hasta que logren ejecutar de manera independiente el diagnóstico clínico de las anomalías dentomaxilofaciales.

Durante la planificación se deben determinar las alternativas en correspondencia con los estudiantes, de manera que se diseñen estrategias colectivas, dirigidas al logro de objetivos grupales, y estrategias de atención diferenciada, que se centren en la solución a problemáticas particulares de un subgrupo de estudiantes y de estudiantes específicos, para así lograr una enseñanza con carácter flexible.

Aunque para la enseñanza del diagnóstico clínico de anomalías dentomaxilofaciales se utilizan los escenarios y especialistas de la atención secundaria de salud, se debe contextualizar a la atención primaria de salud, donde el estomatólogo general básico realiza su labor profesional, los instrumentos y materiales de que dispone, así como las relaciones con el resto de los profesionales del equipo básico de salud.

Estos presupuestos son de gran importancia si se aspira a transformar la enseñanza del diagnóstico clínico de anomalías dentomaxilofaciales desde un enfoque tradicional a uno desarrollador.

 


Within the Cuban current educational practice the conditions of the teaching-learning process in the subject of Orthodontics in the Dental career needs the clinical diagnosis of dentomaxillofacial anomalies to be transformed to become systemic, systematic, flexible, personological and contextual. This transformation must be reversed in the organization, implementation and control of the conceptual, procedural and attitudinal contents, for the students to be able to identify and interpret, through the clinical method, the morphological, aesthetic and functional alteration of dentomaxillofacial structures.

To move to the teaching of this content with developing approach, it is not only the transmission of knowledge what should be taken into account but should be considered the unity that in the practice activity have the instrumental-cognitive, the evaluative-affective and the behavioral. The systemic character of the teaching is achieved by linking knowledge, skills, habits, emotions, feelings, motives, interests and metacognitive processes, in order to contribute to instruction as well as as training, to education, and therefore the development of students.
The personological character is achieved by considering the integral educational diagnosis from planning consultation for the allocation of patients to students, so that the complexity of the anomalies can be increased gradually and progressively (syndromes class I, II and III, temporary, mixed and permanent dentition), interpretation of ancillary tests and the degree of cooperation of the patients and their families. Differentiated actions should be performed, allowing the transit of the current development area to the potential one. The educational diagnosis is systematically updated according to the transformation achieved in students and the group.
In order to achieve the systematic character of this teaching, actions should be taken from the beginning of the course. Content of craniofacial growth and development as well as the teeth and occlusion, can be used to teach to interrogate and examine. The teaching of the rest of the content should be continued, departing from the teacher demonstration and student continuous exercise with new patients until they can independently run clinical diagnosis of dentomaxillofacial anomalies.
During the planning alternatives should be determined in correspondence with students, so that collective strategies can be designed, directed to the achievement of group objectives, and strategies of differentiated services that focus on solving a particular problem for a subgroup of students and specific students in order to achieve flexible teaching character.
Although for the teaching of clinical diagnosis of dentomaxillofacial anomalies the scenarios and specialists of secondary care are used, it should be contextualized the primary health care, where the basic general dentist performs his professional work, tools and materials available and the relationships with other professionals in the basic health team.

These budgets are very important if you want to transform the teaching of clinical diagnosis of dentomaxillofacial anomalies from a traditional approach to one significant.
